COQUITLAM — Another patient has gone missing from the Forensic Psychiatric Hospital in Port Coquitlam.

Matthew David Mikolajczyk, 29, was reported missing from the hospital as an unauthorized absence, Thursday.

Coquitlam RCMP issued a Canada-wide warrant for the missing man.

Mikolajczyk is described as standing six feet, one-inch tall, 170 pounds, with a slender build, short brown hair and a shaggy beard.

He was last seen wearing a dark blue, long-sleeved shirt and dark blue sweat pants.

Police are warning the public not to approach the missing man, but rather call 9-1-1 if they spot him.

This is not the first time Mikolajczyk has escaped from the hospital.

He went missing from the hospital in 2009 and in 2008.

In 2008, police captured Mikolajczyk on Saturna Island following a four-hour search.

Mikolajczyk was previously found guilty of assault, willfully resisting or obstructing a police officer and breach of probation in cases dating to 2004. In the past, he has also been found not criminally responsible, due to a mental defect, and unfit to stand trial.

Earlier this year, two patients from hospital went missing during unescorted day-passes.

David Fomradas, 34, and Gregory Owen Schleen, 49, both went missing from the hospital within days of each other back in May.

Schleen was later returned in August after being picked up by police in Vancouver, while Fomradas remains on the lam.

The pair's disappearance sparked two reviews of the hospital's unescorted day-pass program and an outcry from politicians and residents in the community.
